Extracorporeal Shockwave Lithotripsy (ESWL)

Non-Invasive Treatment for Kidney Stones

At Fore River Urology, we offer Extracorporeal Shockwave Lithotripsy (ESWL) — a safe, effective, and non-invasive procedure to treat certain types of kidney stones. Using targeted sound waves, ESWL breaks stones into smaller pieces, allowing them to pass naturally through the urinary tract.

If you’re dealing with kidney stone pain or have been diagnosed with stones that are not passing on their own, ESWL may be the right solution.

What Is ESWL?

ESWL stands for Extracorporeal Shockwave Lithotripsy. It is a non-surgical treatment that uses high-energy sound waves (shockwaves) to break kidney stones into small fragments. These fragments can then be passed naturally through urine, often with much less discomfort.

  • Non-invasive: No incisions, scopes, or hospital stays
  • Outpatient procedure: Most patients go home the same day
  • Minimal recovery time

Who Is a Candidate for ESWL?

ESWL is often recommended for patients with:

  • Stones located in the kidney
  • Stones that are less than 2 cm in diameter (or <1cm in the lower pole)
  • Stones made of materials that fragment well
  • Minimal or no obstruction of urine flow

Our providers will evaluate your imaging and symptoms to determine if ESWL is the right approach.

How the Procedure Works

  • You’ll lie on a padded table while a specialized machine delivers focused shockwaves to your kidney.
  • These waves pass through skin and tissue without harming them, targeting the stone directly.
  • Most procedures take 45–60 minutes.
  • General anesthesia is used to help you remain still and comfortable.

After the Procedure

  • Most patients resume normal activities within a day or two
  • You may notice blood in the urine, mild discomfort, or the passage of stone fragments over several days
  • We recommend increased fluid intake to help flush out debris
  • A follow-up imaging study is often performed to confirm stone clearance

Benefits of ESWL

  • No incisions or scopes
  • Shorter recovery compared to surgery
  • Effective for many types of stones
  • Outpatient convenience

Limitations of ESWL

  • May not be effective for larger, hard, or ureteral stones
  • Some patients may require more than one treatment
  • Not recommended for pregnant women or patients with bleeding disorders

Potential Complications:

  • Hemorrhage (perinephric hematoma)
  • Hypertension (from Page Kidney)
  • Steinstrasse (German for ‘stone street’ where fragments become log-jammed while trying to pass)
  • Pain
  • Infection
  • Insufficient stone fragmentation
